Your rights under HIPAA
Under the Health Insurance Portability and Accountability Act (HIPAA), you have the right to:
- Request a copy of your medical record
- Request corrections to your medical record
- Request confidential communication
- Ask us to limit what we share
- Get a list of disclosures we have made
- Get a copy of this privacy notice
- Choose someone to act for you
- File a complaint if you believe your privacy rights have been violated
How we use and disclose your information
We may use and share your information for:
- Treatment. Your counselor may use your information to provide care and may share it with other providers involved in your care, with your written authorization.
- Payment. We may share information with insurance carriers and billing services to get paid for services provided.
- Operations. We may use your information to improve our services, train staff, and conduct quality reviews.
- Court-referred programs. Attendance and completion information for court-approved programs may be shared with the referring party per your written authorization.
Required disclosures
Counselors are legally required to disclose information in certain circumstances, including:
- Imminent risk of harm to yourself or others
- Suspected child abuse or elder abuse
- Certain court orders or subpoenas
- Public health reporting requirements
Secure communication
We do not transmit Protected Health Information over non-secure email. For anything involving your clinical care, please use our secure patient portal or call our office.
Complaints
If you believe your privacy rights have been violated, you can file a complaint with us or with the U.S. Department of Health and Human Services Office for Civil Rights. You will not be retaliated against for filing a complaint.
Effective date
This notice is effective as of April 21, 2026. We may update this notice from time to time. The current version will always be posted here.
