Your privacy rights at a glance
If you live in California, Colorado, Connecticut, Virginia, Utah, Texas, Oregon, or Montana, you have specific rights under your state's privacy laws. This page describes those rights and how to exercise them. If your state is not listed, you may still have similar rights — call us at 501-464-2926 and we will help.
Categories of personal information we collect
- Identifiers: name, email address, phone number, postal address.
- Contact and inquiry data: any message or form you submit through this site.
- Internet activity: pages viewed, referring URL, device type, IP address (via Google Analytics, with your consent).
- Health information: only if you provide it directly through our patient portal — that data is HIPAA-protected, not handled under state privacy law.
We do not sell personal information. We do not share personal information with third parties for cross-context behavioral advertising.
Your rights and how to exercise them
- Right to know. Request the categories of personal information we have collected about you, the sources, the purposes, and any third parties we share with.
- Right to access. Request a copy of the personal information we hold about you.
- Right to delete. Request deletion of personal information we hold about you, subject to certain HIPAA and state-law retention requirements.
- Right to correct. Request correction of inaccurate personal information.
- Right to opt out of sale or sharing. We do not sell or share personal information for cross-context behavioral advertising. If our practices change, we will update this page and offer an opt-out link here.
- Right to limit use of sensitive personal information. Health information you submit through our patient portal is HIPAA-protected and used only for treatment, payment, and operations as described in our HIPAA Notice.
- Right to nondiscrimination. We will not deny services, charge different prices, or provide a different level of service because you exercised your privacy rights.
- Right to appeal. If we deny a request, you may appeal — contact us using the methods below.
How to submit a request
Send a verifiable request by any of these methods:
- Email: admin@arcounselingandwellness.com (subject line: "Privacy Request")
- Phone: 501-464-2926 during admin hours (Monday through Thursday, 9 AM to 2 PM Central)
- Mail: Arkansas Counseling and Wellness Services, 4702 W. Commercial Dr., Suite B3, North Little Rock, AR 72116. Mark the envelope "Attn: Privacy Request."
We will verify your identity by matching the contact information you provide against records you authorized us to keep. We respond within 45 days; for complex requests we may extend by up to 45 additional days with notice.
Authorized agents
You may authorize a trusted person to make a request on your behalf. We will verify your identity and the agent's authorization. To authorize an agent, send a signed, dated permission letter using one of the contact methods above.
Cookie controls
Our consent banner lets you choose which categories of cookies and tracking are active. To change your choices, click the cookie-control link in our footer or clear your browser cookies for this site, then reload.
Children's privacy
This site is not directed to children under 13. We do not knowingly collect personal information from children. If you believe a child has submitted personal information through our site, please contact us so we can delete it.

We will post any changes to your privacy choices on this page. The "Last updated" date at the top reflects the most recent revision.
